Keio Application Required Documents
After receiving your nomination from the MIX office, complete your Keio App.
2016 Spring Admission Application Deadline: October 5, 2015. This date is according to Keio's website; however, the MIx office just told us to turn in our documents ASAP. It is hard getting all these documents done in a week, which is why we were told that it is ok to turn in after the deadline, as long as we get the docs in as soon as we can.

Below is a list of application documents for KIP (Keio International Program):
- Official nomination letter (Cover Letter) from your home institution
- Application form (with photos)
- Study proposal
- This includes what classes you plan to take so be sure to research their course catalogue and see which credits transfer back to your home institution.
- One academic reference
- I recommend asking professors for this weeks in advance (must be written within the past 3 months). Don't wait for your nomination to start thinking about letters of recommendation.
- Self-evaluation sheet for Japanese language proficiency
- Official transcript
- For your reference regarding transcript requests from UH Office of the Registrar:
- Standard request: $5 per copy mailed out or available for pick-up within five business days of receiving request
- Rush Request: $15 per copy mailed out or available for pick-up within two business days of receiving request
- For your reference regarding transcript requests from UH Office of the Registrar:
- Certificate of health
- You need to have this filled out by your doctor
- I had to drop off the form and get my TB test done on a Friday and then go back the next Monday to get the completed form and results of the TB test.
- Certificate of financial support
- This will be an official letter from your bank, a letter from your parents saying they will financially support you as well as a letter from your parent's bank, or a letter from the scholarship/organization stating the amount of funds that is to be provided. (See Keio website for more details)
- One copy of passport
- You should also make a second copy to keep for your records in case of emergency
- One photocopied set of documents #2-7 (stapled together at the top left of page)
Also don't forget:
- Online registration (you should actually do this first)
- Print 3 or more photos of yourself (3cm x 4cm, glossy) and paste one to your application form, attach two others to the form
